Services offered:
Free Occupational and Physical Therapy
- Evaluation & Treatment
- Evaluation for assistive device & bathroom equipment
- Walkers, shower chairs, etc. may be available free or low cost
- Offered once a month at the Mazie Woodruff Center
- Referrals to other services as needed
Home Safety Inspection and Repairs
- Home modifications for increased function
- Examples of modifications: Ramps, widening doorways, & making room for walkers or wheelchairs – free to low cost
- Necessary home repairs at low cost
Mobile Unit offers Fall Risk Screeining
- Free balance and strength testing
- Information on how to reduce risk of falls
- Referrals to other resources needed
Who to Refer
- Individuals who live in neighborhoods in East Winston or Northside & are 45+ years
- Patients with deficits in balance, strength, activity performance, and/or decline in function Patients who will benefit from education in maintaining current level of function
- Patients who are at risk for falls or decline
- Patients who are aging and will potentially need home-modifications for safety purposes, such as walk-in showers and ramps to home, or equipment-such as shower chairs & wheelchairs.
